- Submit the following duly signed and sealed by Professional Electrical Engineer (PEE) and approved by the Municipal Engineer.
- a) Electrical Plan (Municipal Engineering Office)
- b) Application for Electrical Permit (Municipal Engineering Office)
- c) Certificate of final inspection (Municipal Engineering Office)
- d) Electrical Permit (Municipal Engineering Office)
- Business Permit (for commercial only) (Municipal Office)
- Fire Clearance (exclusion light materials) (Municipal Office)
- Membership Application
- a) Three copies of Job Order prepared by authorized electricians of BANELCO or (Licensed Electrical Engineers/Professional Electrical Engineers/Master Electricians). (Accredited Electrician)
- Valid I.D. of the applicant and photocopy of the valid I.D. with signature of the applicant.
- Series agreement duly notarized only to series connections. (only if “layo” sa poste).
- Lot Consent (Accredited Electrician) OR Deed of Sale OR Tax Declaration duly notarized.

1.1 DEED OF SALE or TAX DECLARATION which is already under the NEW owner’s name. | |||

2.2 IF ONE OF THE IMMEDIATE FAMILY will take over the property, ALL other immediate family members should sign a waiver notarized by a lawyer. | ||||
2.3 IF NOT MEMBER OF THE FAMILY, waiver should still be signed by all of the immediate family members and must be notarized by a lawyer. | ||||
2.4 Photocopy of the valid ID of both previous and new owner of the property |
