Requirements for Candidacy
- Letter of Intent
- Birth Certificate – (RA 10531, Sec. 26-B)
- Certificate of Candidacy – Notarized (tob be accomplished in four(4) copies)
- Candidate’s Personal Data Sheet (four (4) copies – (Form available upon request)
- Certificate of No Outstanding Account in the last five(5) years (pursuant to Sec. 26-B(e), RA 10531 Certified by Service Area, with the concurrence of Finance manager – 3 copies)
Any of the following (for purposes of good moral character):
- Barangay Clearance
- NBI Clearance
- Police Clearance
- Certificate from Religious Sect Leader where the candidate is affiliated
Additional Requirements:
- COMELEC Clearance (pursuant to Art. II, Section 7.14 of 2005 NEA Guidelines)
- Fiscal Clearance (pursuant to Art. II, Sec. 11.2 a.6, 2005 NEA Guidelines)
- Income Tax Return (Latest)
- ID Picture (lates 2×2 Photo – 2 pcs.)
- Membership Record and Certification
- Certificates of Attendance during AGMA in the last five(5) years preceding the election (pursuant to Sec. 26-B, RA 10531 – to be secured from ISD)
- Transcript of Record of a four (4) year course(pursuant to Sec. 16-B (b),RA 10531)
- College Diploma and Licensure, if any
- Certificate of Employment Clearance from Employer (resignation, retirement, separation, etc.) indicating no accountablilities and administrative/criminalcivil cases(Sec. 15(d),IRR)
- Spouse’s Personal Data Sheet (if employed with the government/GOCC, indicate position and salary grade), (Sec. 15 (a),IRR)
- Affidavit RE: No relationship within the fourth civil degree of consanguinity or affinity to any member of the EC Board of Directors, Department Managers, NEA-appointed Project Supervisor (PS) or Acting General Manager (AGM) and its equivalent or higher position (Sec. 15(f) IRR).
