REQUIREMENTS FOR NEW SERVICE CONNECTION

  1. Attend pre-membership seminar every Friday 9:00 AM (for residential only).
  2. Submit the following duly signed and sealed by Professional Electrical Engineer (PEE) and approved by the Municipal Engineer.
  3. a)  Electrical Plan (Municipal Engineering Office)
  4. b)  Application for Electrical Permit (Municipal Engineering Office)
  5. c)  Certificate of final inspection (Municipal Engineering Office)
  6. d)  Electrical Permit    (Municipal Engineering Office)
  7. Business Permit (for commercial only) (Municipal Office)
  8. Fire Clearance (exclusion light materials) (Municipal Office)
  9. Membership Application
  10. a)  Three copies of Job Order prepared by authorized electricians of BANELCO or (Licensed Electrical Engineers/Professional Electrical Engineers/Master Electricians). (Accredited Electrician)
  11. Valid I.D. of the applicant and photocopy of the valid I.D. with signature of the applicant.
  12. Series agreement duly notarized only to series connections. (only if “layo” sa poste).
  13. Lot Consent (Accredited Electrician) OR Deed of Sale OR Tax Declaration duly notarized.

 

CHANGE NAME REQUEST REQUIREMENTS
  1. SOLD (NAPALIT) PROPERTY
  1.1 DEED OF SALE or TAX DECLARATION which is already under the NEW owner’s name.  
  1.2 Waiver notarized by a lawyer    
  1.3 Attend pre-membership seminar if its first electrical connection of the new owner of the property.  
  1.4 Photocopy of the valid ID of both previous and new owner of the property  
2. DECEASED OWNER
  2.1 Copy of Death Certificate      
  2.2 IF ONE OF THE IMMEDIATE FAMILY will take over the property, ALL other immediate family members should sign a waiver notarized by a lawyer.
  2.3 IF NOT MEMBER OF THE FAMILY, waiver should still be signed  by all of the immediate family members and must be notarized by a lawyer.
   2.4 Photocopy of the valid ID of both previous and new owner of the property
